    What are the side effects of treatment?

    Most doctors use hormone therapy to control the spread of advanced prostate cancer. The most frequent side effects caused by hormone therapy include reduced sex drive, impotence, infertility, and osteoporosis (weakened bones). Erectile dysfunction and infertility occur in most men who undergo hormone therapy. The risk and extent of bone loss depends largely on how long a man receives hormone therapy. In many cases, doctors can recommend ways to combat these side effects.

    When people no longer respond to hormone therapy, doctors may try a chemotherapy drug called Taxotere. Possible side effects of Taxotere include nausea, hair loss, and bone marrow suppression (the decline or halt of blood cell formation). Patients may also develop fluid retention and pain, tingling, or numbness in the fingers or toes.
    Though new, thus far Provenge has been remarkably safe.

    However, clinical trials suggested the treatment might be linked to a slightly increased risk of stroke. Nearly all patients suffer some mild to moderate side effects such as chills, fatigue, fever, back pain, nausea, joint ache, and headache.

    The first cause is hormonal. After about age 40 more of the 5-alpha-reductase enzyme is produced by men. This enzyme converts testosterone, which we all want to have more of as we age, to dihydrotesterone or DHT. As men age, DHT levels tend to raise because of this increased enzyme production. DHT causes the prostate to increase in size. Higher DHT levels are one of the major causes of enlarged prostates. However they are not the only cause. If they were, herbal formulas containing saw palmetto would work better than they do.

    It seems that the majority of men suffering from enlarged prostates are not helped significantly by herbal formulas containing saw palmetto, pumpkin seeds and other herbs which block the formation of DHT and may inhibit its binding to cell receptors in the prostate. Most likely this is for two reasons. First is that these herbs don’t work as powerfully as their promoters would like us to believe. Secondly, they don’t deal with what may be an even more major cause of an enlarged prostate. Infections.

    Our guess is that approximately 2/3 of all men suffering from an enlarged prostate have an infection in the prostate. This infection could be viral, bacterial, fungal or even mycoplasm. Infections tend to cause inflammation. An inflammation in the prostate enlarges it. And of course you may suffer from both an infection and increased DHT levels. Your doctor may be able to give you an antibiotic to help you with a bacterial prostate infection, but he has little to help with viral and mycoplasm infections.

    For years this report has recommended unique products for improving the symptoms of an enlarged prostate. These products worked better than the usual saw palmetto and sterolin based herbal formulas. Because they helped many people that were not helped by these usual types of herbal formulas. The first product was U-Fn, a u-fucoidan extract from brown algae that was expensive but better than the herbs.

    Unfortunately, many people still were not helped. So a few years later we heard about an oxygenating, detoxifying, pH balancing liquid supplement that fights infections. Zeo 3. Not only was it a bit more effective at helping people deal with a swollen prostate, but was less expensive too. However, it seemed to help just about 60% of the men using it.     For most patients suffering from an enlarged prostate who do not have kidney damage or serious infection, the ultimate decision must be left to the patient. It’s approved only for treatment of patients with few or no symptoms who have prostate cancer that has spread outside the prostate and no longer responds to hormone therapy.

    These cannot prevent the decline in testosterone levels, but they can lead to an elevated rate of transformation of testosterone into 5-alpha-dihydrotestosterone (DHT) and/or to the increased binding and/or to the decreased clearance of DHT from prostate cells.     Be wary of spicy foods. Spicy and acidic foods bother some men with enlarged prostates, says Dr. Neal. “”If you notice more problems after eating salsa, chili or other spicy or acidic foods, then you’re among those men–and you should avoid that cuisine.”"

    Manage your stress.     Saw palmetto is the leading herbal treatment for BPH and among the best selling of all herbal products, with annual U.S. sales of more than $25 million, according to the American Botanical Council.

    “”Saw palmetto is usually the first line of therapy against BPH at least by patients or their spouses,”" quips Franklin Lowe, MD, MPH, associate director of the Department of Urology at St. Luke’s/Roosevelt Hospital and chairman of the American Urological Association’s Complementary & Alternative Medicine Committee. “”Most patients are taking it before they come to their urologist’s office to find out if they should be taking it.”" Still, many urologists endorse it use for patients with early, troubling symptoms.

    Pros: This herb brings relief in about one in three patients, with a suspected effect similar to Proscar, by blocking the production of a hormone that causes the prostate to grow. “”Nobody really knows exactly why it works, because studies are conflicted on the suspected mechanism,”" says urologist Eric K. Seaman, MD, FACS. “”What is clear is that it helps a lot of people, and there is no documented evidence of any side effects. I know a urologist who was waking up at least three times a night because of BPH, and since taking saw palmetto, he’s up only once a night. So even if the majority of patients don’t get a benefit, they don’t lose anything either other than a few dollars.”

    Cons: One possible explanation for why the majority of patients don’t get relief? The active ingredient of saw palmetto like its exact mechanism hasn’t been identified, so different formulations may produce different results, says Katz.

    Red clover has long been used as an alternative treatment for a variety of conditions from menopausal hot flashes to psoriasis because it is rich in isoflavones, a protein found in soybeans. Not only is this believed to help inhibit prostate and other cancer tumors, it could help treat BPH. A product commercially sold as Trinovin (and whose manufacturer paid for Katz’s study) has been found in several trials to improve urine flow and decrease symptoms by as much as 23%.

    Pros: Isoflavones are suspected of helping more than BPH, but Trinovin contains four types believed to be powerful for overall health and in treating BPH and reducing prostate cancer risk.
    Cons: Some studies suggest that improvements are most noticeable after one month and then continue at a much slower rate. Little or no improvement is noticed at varying doses.
    Soy contains high concentrations of the same isoflavones found in red clover and is often cited as a reason why Japanese men rarely get prostate cancer.

    Pros: Also believed to offers many health benefits and strong evidence of reducing prostate cancer.
    Cons: For maximum benefit, it’s recommend that men consume eight ounces of soy foods daily, which is impractical and would create severe gas problems, says Katz.

    African tree bark (pygeum africanum) is so popular in Europe and Africa that the tree from which this treatment is harvested is now on the endangered list.

    Pros: Studies show it brings relief, but how? Some show it relieves symptoms by reducing prostate swelling, while others indicate it may reduce inflammation or inhibit growth factors associated with prostate enlargement and tumor development.

    Cons: Typically not a standalone treatment, and usually used in conjunction with saw palmetto.
    Of course, because they are not regulated by the FDA, some experts say that many herbal supplements vary widely in the amount of active herbal extract claimed on the label. A study presented to the American Urological Association last year found that some brands tested contained less than 15% of the active ingredient listed on their label.

    1. HORMONIC THEORY: As age advances , male hormones diminish, while estrogen does not. Tissues are enlarged due to predominance of estrogenic hormone. This is involuntary hyperplasia like fibro adenoma of breast.

    PATHOLOGY:
    Prostate enlargement is strongly related to normal aging. Some of the factors involved are quite well understood. Nevertheless, there also is substantial disagreement about other issues. BPH can be called an aspect of male menopause because an increased ratio of estrogen to testosterone is active in BPH just as, conversely, in women passing through menopause the ratio of testosterone to estrogen increases. It is generally accepted that hormone ratios and hormone clearance are involved in BPH, but the exact ways in which these lead to the enlargement has yet to be definitively explained. Testosterone, the “male” hormone, is at its peak during adolescence. It decreases thereafter, and the rate of decrease sharpens by about age 50. The decline in testosterone production typically calls into play the compensatory release of other hormones which are stimulants to testosterone production.
